To Individuals and Families supported by Care Design NY:


We are honored that you have chosen Care Design NY (CDNY) as your Care Coordination Organization/Health Home.

Individuals and families are at the center of all we do at CDNY. The development of Individual/Family Regional advisory boards is essential to the operational direction and evaluation of the quality of CDNY services, in its efforts to achieve its organizational mission of assisting individuals with intellectual and/or developmental disabilities to get the supports and services they need to live a quality life.

We are currently in the process of forming an Individual/Family Advisory Board in your region. This will provide the opportunity for individuals and parents to directly engage and garner feedback from others we are supporting through care management services. Each Advisory Board will consist of 15 members that will actively participate in regularly scheduled meetings in the region.

The Advisory Boards will focus on several key areas including: the hiring, training and evaluating of the workforce; quality and metric development communication and outreach (including the educational system); and assisting CDNY in resolving regional issues and any other areas of topics determined by the Regional Board.

We welcome individuals/family members, who we support, to volunteer to serve on these Advisory Boards. Reasonable accommodations will be made as needed for individual and family Advisory Board members to maximize their attendance and participation.

Attached you will find materials, including a letter of introduction and brochure, for you to review regarding the development of the Advisory Boards.

I’d also like to introduce you to Danielle Wiltsie, CDNY’s Director of Individual, Family and Advocate Supports. Danielle, who began working at CDNY in August is excited about the work we will be doing together with individuals and families. Danielle will be another “go to” person at CDNY and will be coordinating the development of and support to all the Individual/Family Regional Advocacy Boards.

Regional Advisory Boards

Up to 7 Regional Boards covering Long Island; New York City; Hudson Valley; Capital District and North Country (15 board members each)

Living Arrangement (certified home, with family, on own)

  • Regional Board Meetings to occur on a regular basis but no less than quarterly.
  • Some Regional Board meetings will be open meetings to allow input and to discuss specific topics with individuals and families from that region.
  • Charters to be developed by the Boards and will cover such areas as:
    • How members and the chairperson are selected and their teams
    • Workforce: hiring; training; evaluating
    • Quality including individual and family perspective on metrics
    • Communication and information sharing
    • Community and school outreach
    • Survey-evaluating performance-feedback
    • Brainstorm on synthesizing regional issues and developing proposed solutions
  • CDNY staff to attend and participate in regional meetings with agenda set by Board.
  • Regional Board representatives to periodically attend CDNY Executive Staff meetings.
  • Reasonable accommodations will be made as needed for individual and family. Advisory Board members to maximize their attendance and participation.

Care Design NY Emergency After Hours On-Call

Please only use number for urgent matters that cannot wait until the next business day.

The emergency after hours number will only be in operation after 5pm to 8:30am on business days and 24 hours per day on weekends and Care Design NY holidays. Please use our office lines during normal business hours.

Care Design NY Compliance Hotline

This number is to be used by anyone to report a suspected violation of the code of conduct, compliance guidelines, operational policies, a law or regulation, or other concern to the compliance offices.
