Topics will include Transition Planning, Supplemental Nutrition Assistance (SNAP), Medicare and Medicaid Benefits, Keeping Benefits While Working, Completing Medicaid Applications and Re-certifications, and more. The leadership of Care Design NY holds periodic meetings in each region to share information with individuals and families. These Family Forums are an opportunity to meet with our leadership in person and ask any questions you may have about Care Design NY, your care manager, access to services in your region, and any changes happening in New York that effect individuals with intellectual and/or developmental disabilities. During the Family Forums, we will present and new information we have and then take questions. We are always interested in hearing feedback about the supports we are providing to you and your loved one, and we are open to suggestions for how we could improve.
