1 Year Review

Our Story

Care Design NY was created with the involvement and participation of over 70 agencies with long and successful histories of specializing in the provision of services to children and adults with Intellectual and/or Developmental Disabilities (IDD). Our story is rooted specifically in the provision of Medicaid Service Coordination.

The agencies recognized that with the requirement for New York State to provide Medicaid Service Coordination in a conflict-free way, and to increase the provision of that service to a more elevated “Care Management," they wanted to be involved and on the forefront of that transformation.

We are stronger with the involvement of all of our partners. One in particular, Partners Health Plan (PHP), is the only managed care health plan for individuals with IDD in New York State. 

Our Mission

Our mission is to assist individuals with intellectual and/or developmental disabilities to get the supports and services they need to live a quality life.

Our Commitment

Individuals and families are at the center of all we do at Care Design NY. We will assist you, and advocate with and for you, to ensure that you have information and resources to make informed choices in your life.

We will work in partnership with you to achieve the life you desire, ensuring that you have access to the support and services that are needed by providing Health Home Care Management. This newly designed service is a new way to coordinate care, combining developmental disability services and supports with health, wellness and behavioral health services.

Each of our member has affiliated agencies that have a history of providing supports and services to individuals with intellectual and/or developmental disabilities.

Transition Plan

for Home and Community-Based Services (HCBS), Health Home Care Management for Individuals with Intellectual and/or Developmental Disabilities (I/DD), and the Development of Specialized Managed Care
With Federal and State approval, beginning July 1, 2018, New York State initiated the transformation of the State's system of services for people with intellectual and/or developmental disabilities (I/DD) to better integrate services, promote the better use of resources to meet growing and changing needs, and become truly person-centered.

Rest assured that Care Design NY will help you; we will advocate with you, and for you, to have access to the most integrated services and all the information you need to make informed choices for a meaningful life.

A Message from our CEO

Dear Members and Family Advocates,

As we start 2019 in our 7th month of operation at Care Design NY, I’d like to highlight our efforts to date and what you should expect from us in this new year.

Our First Seven Months

  • The transition from Medicaid Service Coordination (MSC) to Care Management went about as smoothly as one could expect with a service change; new organizations providing the service; staff needing to learn new systems, etc. There have been bumps in the road, but we have worked hard to minimize them.
  • We were very successful in our goal to maintain existing relationships between individuals and family advocates and their care managers wherever possible.
  • We demonstrated to our workforce, especially our Care Managers, that they are our greatest asset and that we are here to support and invest in them in every way possible. Our total compensation and time-off package is very competitive. Our training and communication efforts have been frequent and robust. I believe that, if our workforce feels appreciated and supported, they will work very hard to deliver on our mission to assist individuals with intellectual and/or developmental disabilities to get supports and services to live a quality life.
  • We began developing our own culture at Care Design NY -- one of openness, transparency, responsiveness, and professionalism.



Our Focus for 2019

  • Support our workforce who support our members and their families.
  • Complete the transition to Life Plans for all members.
  • Work closely with our regional Advisory Boards, made up of individuals and family advocates, to identify and solve problems and to make Care Design NY a better organization.
  • Continue to foster communication with our members and family advocates by holding information sessions this winter and again in the fall.
  • Begin to hire self-advocates and family advocates as ombudsman within Care Design NY to support other self-advocates and family advocates, as well as to work with our other staff on various initiatives.
  • Begin to identify, and problem-solve, service gaps for our members with service providers and fiscal intermediaries.

It’s going to be a busy year, and we look forward to continuing to work with individuals and families to get the supports and services they need to live a quality life.
